Health Care Quality Report Card
The California Health Care Quality Report Cards show the quality of health care for millions of Californians who get their care through medical groups and commercial insurance plans. Quality health care is getting the right care at the right time. * The Health Plan Report Card data includes the 10 largest Health Maintenance Organizations (HMOs) and the 5 largest Preferred Provider Organizations (PPOs). * The Medical Group - Commercial Report Card data includes 204 medical groups that have contracts with the HMOs listed in the Health Plan Report Card. * The Medical Group - Medicare Report Card data includes 186 medical groups that have contracts with the Medicare Advantage plans listed in the Health Plan Report Card and serve Medicare Advantage members. * The Counties data lists the counties served by each health plan and medical group included in the Report Cards. This dataset includes the number of eligible individuals selected and enrolled in a Covered California qualified health plans (QHPs) by rating region and by reporting period. California is comprised of 19 rating regions, and each region has different pricing and health insurance options. Covered California reported data is from the California Healthcare Eligibility, Enrollment and Retention System (CalHEERS) and includes eligible individuals who selected and enrolled in a QHP, and paid their first premium. This dataset is part of public reporting requirements set forth by the California Welfare and Institutions Code 14102.5.

CA Wellness Plan Data Reference Guide

The purpose of the California Wellness Plan (CWP) Data Reference Guide (Reference Guide) is to provide access to the lowest-level data for each CWP Objective; lowest-level data source, instructions to access data, and additional details are described. Some CWP Objectives do not have program leads, data sources, baselines, and/or targets, but are included because they were a result of CDPH program or partner input and were felt to be important to the reduction of chronic disease incidence, prevalence, and health disparities. Agencies, programs and/or partners identified with an objective may be either data stewards and/or engaged in activities to achieve the target, but may not have adequate resources for statewide activities. Developmental Objectives will be updated as information becomes available. Background: The California Wellness Plan, California's Chronic Disease Prevention and Health Promotion Plan was released February 2014 by the California Department of Public Health (CDPH). The overarching goal of CWP is Equity in Health and Wellbeing; additional CWP Goals include: 1) Healthy Communities, 2) Optimal Health Systems Linked with Community Prevention, 3) Accessible and Usable Health Information, and 4) Prevention Sustainability and Capacity. All CWP objectives fall under the framework of Let's Get Healthy California Task Force priorities. County Health Status Profiles

County Health Status Profiles is an annually published report for the State of California by the California Department of Public Health in collaboration with the California Conference of Local Health Officers. Health indicators are measured for 58 counties and California statewide that can be directly compared to national standards and populations of similar composition. Where available, the measurements are ranked and compared with target rates established for Healthy People National Objectives. For tables where the health indicator denominator and numerator are derived from the same data source, the denominator excludes records for which the health indicator data is missing and unable to be imputed. The Workforce & Quality Incentive Program Quarterly Performance Reports (WQIP) contain the following WQIP metrics: acuity-adjusted staffing, staffing completeness, staffing turnover, MDS clinical metrics, MDS data completeness, claims-based clinical metrics, Medi-Cal disproportionate share metric, and MDS racial and ethnic data completeness. The data set will include the distribution of facility-level scores for the currently available performance period data, as well as a comparison to the benchmark values for the MDS clinical and acuity-adjusted staffing hour metrics. California Hospital Inpatient Mortality Rates and Quality Ratings

The dataset contains risk-adjusted mortality rates, quality ratings, and number of deaths and cases for 6 medical conditions treated (Acute Stroke, Acute Myocardial Infarction, Heart Failure, Gastrointestinal Hemorrhage, Hip Fracture and Pneumonia) and 5 procedures performed (Abdominal Aortic Aneurysm Repair, Unruptured/Open, Abdominal Aortic Aneurysm Repair, Unruptured/Endovascular, Carotid Endarterectomy, Pancreatic Resection, Percutaneous Coronary Intervention) in California hospitals. The 2022 IMIs were generated using AHRQ Version 2023, while previous years' IMIs were generated with older versions of AHRQ software (2021 IMIs by Version 2022, 2020 IMIs by Version 2021, 2019 IMIs by Version 2020, 2016-2018 IMIs by Version 2019, 2014 and 2015 IMIs by Version 5.0, and 2012 and 2013 IMIs by Version 4.5). The differences in the statistical method employed and inclusion and exclusion criteria using different versions can lead to different results. Users should not compare trends of mortality rates over time. However, many hospitals showed consistent performance over years; “better” performing hospitals may perform better and “worse” performing hospitals may perform worse consistently across years. This dataset does not include conditions treated or procedures performed in outpatient settings.
