FHI360 was awarded a five-year grant by USAID to implement a project with the following goals: 1) increase the availability of high quality comprehensive care and treatment services for PLHIV at community level, and (2) to strengthen community-level health systems to monitor, track and maintain PLHIV in care. The core activities in this project are household index case testing, linkage to care, defaulter tracking and identification and formation of community ART refill groups as part of differentiated care model. The data set summarizes the project's achievement from October 2015 through September 2016. It highlights the data elements which the program tracked during this period.
