The Child Deaths 2013 Annual Report is Family and Community Services’ (FACS) fourth public report examining our involvement with the families of children and young people who died and were known to FACS. By providing contextual information about the deaths of children who were known to FACS, this report aims to provide learning opportunities for all practitioners who work with children, whether they are within the child protection system or providing other services to families. The report also aims to enhance the public’s understanding of the complexities of child protection work and the underlying factors of disadvantage that affect so many families in NSW.

This is volume 1 of our annual report on reviewable deaths in 2012 and 2013. It documents our review of the deaths of 41 children in 2012 and 2013, and a separate review of 83 children who were killed in circumstances of familial abuse over the 10 year period to 2013. The Ombudsman is required to review these deaths under Part 6 of the Community Services (Complaints Reviews and Monitoring) Act 1993.

The Report of reviewable deaths 2014 and 2015 considers the ‘reviewable’ deaths of 54 children that occurred in NSW in those two years. The deaths were reviewable because the child died in circumstances of abuse or neglect, or in suspicious circumstances, or the child was in care at the time of their death. Included in the report is our review of a vulnerable group of young people in care who died by suicide or in risk-taking circumstances, and a review of 124 neglect-related deaths that occurred in NSW over a 10 year period from 2006 to 2015. Separately to our report, the Reporting of Fatal Neglect in NSW is the result of work we commissioned from Dr Deborah Scott at the Australian Institute of Family Studies. Dr Scott reviewed our reporting of fatal neglect in relation to reporting in other States and countries.
