The Employee Survey question relating to this performance measure: “Please rate your level of agreement: My physical work environment (building) is safe, clean & maintained in good operating order.” Survey respondents are asked to rate their agreement level on a scale of 5 to 1, where 5 means “Strongly Agree” and 1 means “Strongly Disagree” (without “don’t know” responses included).,The survey was voluntary and employees were allowed to complete the survey during work hours or at home.
